Who is this Service for?
Our group home services are ideal for:
- Have been diagnosed with an intellectual or developmental disability (e.g., autism spectrum disorder, Down syndrome, cerebral palsy)
- Benefit from 24/7 support in daily living while still maintaining as much independence as possible
- Thrive in small, supportive group settings with routines, life skills training, and community interaction
- May no longer be best served at home or are transitioning from institutional care or foster settings
- We also work closely with families, guardians, and case managers to ensure a smooth transition and continuous, transparent support.
What we provide
Each Grace Line group home is a thoughtfully managed, family-style residence designed to foster independence, safety, and well-being. Our services include:
- Behavioral and emotional support, including coordination with clinical professionals as needed, is tailored to each client’s individual preferences.
- Personalized daily living assistance (e.g., hygiene, cooking, medication reminders, mobility)
- 24/7 Supervision & Support from trained Direct Support Professionals (DSPs)
- Skill-building programs to promote independence (budgeting, cooking, cleaning, communication, and social skills)
- Community integration and recreational outings that build social confidence and personal enrichment
- Private or shared bedrooms in welcoming, clean, and secure residential homes
- Nutritious meals tailored to individual dietary needs
- Transportation for medical appointments, vocational training, or day programs
- Person-centered planning and individualized service plans (ISPs)
