Supported Living Services(DDA)
We provide seamless support services for our participants in their own homes from early adulthood through retirement and beyond, creating an enabling atmosphere for growth, development, and independence.
Personal Support Services (DDA)
As DDA providers, we understand what it means for individuals with intellectual and developmental disabilities (IDD) to live in homes of their choice, be it their home, care provider’s house, or family home. In any of these circumstances, our services are intended to promote choice, flexibility, self-determination, inclusion, and personal satisfaction.
These Services can include, but are not limited to:
- Assistance with ADL
- Skill building for community integration and independent living
- Assistance with meal preparation and shopping
- Medication Administration and Nursing Support
- Transportation assistance for medical appointments
- Recreational supports
Community Development Services (CDS, DDA)
- Encourage local community events.
Provide resources for travel and transportation - Provide resources for Self-advocacy
- Activities aimed at facilitating and promoting integration and inclusion in the participant’s chosen community
- Provide individuals the opportunity to Volunteering within a non-profit organization whose mission the participants support
- Provide individuals the opportunity to perform a paid or unpaid internship.
Nursing Services (DDA)
Our Registered and Delegating Nurses (RN/DN) are committed to providing custom nursing duties to participants we serve by conducting comprehensive assessments, periodic reviews of medication administration, review of HRST, etc. These focus on participants’ health and safety, thereby preventing practices with adverse outcomes.
